Stepping on the bony top of any foot with percussive type force is very painful. The bones are small and not much padding there. The problem is is that you would need the presence of mind to locate them, then step on them. Remember, you don't have much time in a choke hold situation where the ratero is choking the shit out of you while dragging you back, off balance. If you are passing out and off balance, there's very little chance that this technique will be effective. Another poster mentioned that he went with the mugger, throwing himself backwards, and landing on top of the ratero, giving himself a chance to get away. I believe he said he lost his glasses, but got away, wallet and consciousness intact. Not a bad strategy because you reduce pressure to your neck by "going with the direction" of the attack, or force, and end up on the attacker as he hits the ground hard and braking your fall. Unconventional, but effective in this case. Whatever works.
