Puto has several meanings, all meant to be insulting. When a women calls a man a puto usually she is calling him a slut...meaning that he slepts around with many women. If she wants to call you a homo she would use "maricon". On a general use level it is used as just an insulting word with out any real reference. Just like mean use the word "bitch" to be insulting to women without any real thought.
